About your appointment
A few important things to keep in mind before your appointment:
- if you are a new patient of the practice, please be sure to have obtained a copy of our medical release form, sign it and deliver it to your previous physician’s office if you are transferring care or to your primary care physician’s office if you are seeing Dr. Forbush for a consultation. Also please print and sign our HIPPA consent form and bring this with you to your appointment as well as review our Notice of Privacy Practices
- On our “forms & docs” page, we have directions to our office should you need them
- If you are a new patient of the office, please complete the Pre-appointment questionnaire
- We kindly ask all patients to arrive 10 minutes before their appointment time to complete any necessary paperwork and out of respect for other patients, you will be asked to reschedule your appointment if you arrive more than 15 minutes late
- To meet all of our patients’ need, we require a 24 hour notice for any appointment cancellation. If you do not provide us with a 24 hour cancellation notice, you will be charged a $50 “no show” fee
