Jack Forbush, DO

Dr. Leigh "Jack" Forbush is a graduate of Eastern Maine Medical Center’s Family Medicine Residency Program and Fellowship in Neuromuscular Medicine/Osteopathic Manipulative Medicine having obtained his medical degree at Maine’s prestigious University of New England College of Osteopathic Medicine.

Providing the full spectrum of family medical including Internal Medicine, Pediatrics, Geriatrics and Obstetrics, his particular areas of interest include Integrative and Holistic Medicine, applying the principles of Osteopathy across the age spectrum and the incorporation of information technology in medicine.

Each year, he travels to Paraguay offering his services to the Andrea Ritz Foundation caring for (and enjoying the company of) Guarani of all ages.

He is actively involved at the local, state and federal level in his capacity as the current President of the Maine Chapter of the American College of Osteopathic Family Physicians as well as the President of the New England Direct Primary Care Alliance.

He holds active board certifications in Family Medicine, Neuromuscular Medicine/Osteopathic Manipulative Medicine and Integrative and Holistic Medicine


Ethan Percy, DO

Dr. Ethan Percy is a graduate of Northern Light Eastern Maine Medical Center Family Medicine Residency in 2022. He completed medical school at the University of New England College of Osteopathic Medicine in 2019.

Dr. Percy is trained in full spectrum family medicine and practices osteopathic manipulative medicine.

In his free time, he enjoys spending time with family, hiking, fishing, camping, and snowboarding.

Renee joined out team in 2022 bringing years of Nursing experience to our team including her most recent experience at the St. Joseph’s Hospital Infusion Clinic.

Diana joined us in February, 2022 and brings 20 years of experience in a variety of healthcare environments. Enjoys coffee and a variety of outdoor activities with her 6 children.

Kristin Forbush, LMT

Originally from Connecticut, but a Mainer at heart (although she prefers the sun and surf of Costa Rica).  Kristin manages the business side of the practice in addition to keeping a certain...doctor...in line!

Additionally, she offers a wide range of massage services.


Henrietta "Hank" Poons, LCPC

Henrietta Poons , LCPC is a state licensed clinical counselor with a private practice located within the OCFM office since February 2016. She has worked collaboratively in the primary care setting for over 25 years . “Hank” offers individual, couples and family counseling to address the many psychosocial problems that may affect a person’s well-being. Her practice is open to adults from the community at large as well as patients of the Osteopathic Center for Family Medicine .
